Editing Form Fields
Every Form Field has the following options:
- Label: This will be the displayed name of the field visible to the person filling out the form.
- Width: defines the displayed width of the field.
- Shown to/Required for: The option to choose who the particular field is visible to or required for.
- Specific People: Designates which specific people will have the field be visible/required utilizing either account type and/or tags (set up in 'My Lists')
- Description: Allows for an extended personalized message to appear within the field.
Account Fields
These fields will auto populate for the user when they are logged in and the information is already entered on their accounts. If they do not have that information on their account and they fill it out you can auto-update their account by selecting the 'Apply to Database' option when editing the field:
- Anniversary: The date of the marriage anniversary primary and secondary people on the account.
- Home Phone: (referenced on the accounts as Phone 1) can be found on the account under the People Tab>Edit to the right of the name then the Contact Info tab.
- Marital Status: This field can be set to reference the currently logged in person, Primary or Secondary on the account
- Minyan: This will create a drop down menu to choose from the list of Minyans created in 'My lists'
- Primary Address fields (City, Address 1, Address 2, State and Zip): For the address information on the account not an individual on the account.
- Tribe: Creates a drop down menu to choose from Cohen, Levi and Yisroel (spelling based on site settings)
Enrollment
These fields are used when signing up to pay for an enrollment. The enrollment is selected via drop down menu within each field:
- Enrollment - Choose 1 Radio: Given multiple items within the enrollment selected , all items are shown in the field but only one item can be selected.
- Enrollment - Choose 1 Select: Given multiple items within the enrollment selected, items are selectable via a drop down menu and only the one selected will be visible in the end.
- Enrollment - Choose Multiple: Given multiple items within the enrollment selected, a drop down menu with a check boxes next to each item . Only items selected will be visible in the end.
- Enrollment - Choose Quantity: Given multiple items within the enrollment selected, each item will have a drop down menu with the option to choose from the quantity available from that originally established when the enrollment was created. The remaining slots will also be visible next to the item name i.e. 'item name' 5 of 7 left or 'Item name' 7 open.
Event Seating
These fields are associated with the Event Seating (not Events) chosen in in the drop down menu in form options at the bottom. Only transaction information is saved in the Form submissions, the rest is saved in Event Seating.
These fields function exactly the same way as High Holiday Seating Fields

Financial Fields
These fields draw financial information from an account, public users will not see these fields and they will be skipped. The fields in this section include:
- Donation Based on Contributed Amount: This filed allows you to display what was paid in the past based off of a certain charge type and pay it off if the balance is not $0. If the balance of that charge is $0 because the amount was previously paid off or the account was never charged then a suggested amount box will appear:
First charge type is where payments will go if the total amount is greater than $0.
Date/amount/charge type and payment status are filters used by the form to pull information from the account. Here you can specify a date range amount charged/paid in charge types (specific or all) and whether it was paid in full,partially or not at all. If the account has with the above filters a balance greater than $0 it will clone that amount and add a charge (and ask for payment) under the initial charge type specified above. [Note that any open charges will remain open and a new charge will be created.]
You can then specify if the value is $0 the field will show a new label and a suggested amount that will be paid using a specified charge type. [Note that the amount can be adjusted by the end user and the field can be checked off.] - Require Payment of Due Listed Transactions: This is a required field by default, and displays the open balances of any/all charges on the account within the date range specified. If there is no Balance a suggested amount to a specified charge type.
- Require Payment of Total Due Amount: Functions exactly the same as Require Payment of Due Listed Transactions except instead of displaying each transaction it only displays the total amount.
- Suggest Payment of Due Listed Transactions: Functions exactly the same as Require Payment of Due Listed Transactions except the payments are marked as suggestions and can be edited.
- Suggest Payment of Total Due Amount: Functions exactly the same as Suggest Payment of Due Listed Transactions except instead of displaying each transaction it only displays the total amount.
General
These fields will gather information that the user inputs into the form, and retains that information within the form. It can be viewed in the submission data but will otherwise affect no change anywhere else in ShulCloud.
- Country: Allow for the selection of a country via a dropdown menu.
- Date: Allow for the selection of the date using a visual pop up calendar or typing it in manually. Date will be displayed numerically.
- Date and Time: Allow for the selection of the time and for the selection of the date using a visual pop up calendar or typing it in manually.
- Multiple Checkboxes: This field creates adds a checkable box for any labeled item added to the field, all items are displayed in the field. "Max # checked at once" allows for a limit on checks within a field, additional choices are added by clicking on the blue plus sign on the right of name. This number must be 2 or more, or 0 for no limit. Utilize the Name and Label fields to differentiate between the choices.
- Multiple Lines of Text: A field with a blank text box to allow for the person filling out the field to write in multiple lines of text
- Multiple Lines of Text with Formatting: Same as above with the addition of limited formatting options for the person filling out the form.
- Number: A field that will only capture, numerical value and a dash
- Phone Number: Functions like a single line of text, does not save to/pull from account info.
- Pulldown Selection: Offers unique labeled options in a drop down menu, only one is selectable by the end user.
- Radio Selection (choose 1): Like multiple/single checkboxes shows all options to end user at once, but only one selection possible at a time via, a clickable circle next to each option.
- Single Checkbox: A field that where the label is set next to a single clickable check box.
- Single Line of Text: A field that allows the end user to fill out a single line of text.
- Single Line of Text with Hebrew Keyboard: A field that allows the end user to fill out a single line of text, with the addition of a clickable floating Hebrew keyboard.
- Time: Allows the end user to input a time in a the 12H format, specifying the hour, minute and am/pm.
- US State: Creates a field with a drop down menu of all U.S. states and Washington D.C.
- Website URL: Functions like a single line of text, does not save to/pull from account info.
High Holiday Seating
These fields are associated with the High Holiday Year chosen in the drop down menu in form options at the bottom. All information is saved in High Holiday Seating. Only payment information is saved in form submissions.
The fields in this section include:
- Choose Number of Seats: This field allows the end user to select the number of seats they are choosing.
The Admin can specify a max number of seats selected and choose which locations to be included in the field. If none are selected, all locations will appear, and the form will auto update as more locations are added.
The second dropdown menu is to include any sessions you have set up i.e. Rosh Hashanah day 1/2 and Yom Kippur. If none are selected then all sessions will appear and the form will auto update if more locations are added after.
Actual information will be visible in the Seating Request Block field. - Choose Number Seats with Payment: Functions exactly like the 'Choose Number of Seats' Field but also captures a payment. Only one price can be set up per field so if locations/sessions have different prices, you would need multiple fields (you would only need one Seating Request Block even for multiple fields).
- Seating Request Block: This Field is required to capture names and locations sessions and any custom fields added to the High Holiday seating module. It is recommended to make this field wide, like 800px.
No Data
These fields will not take any information from the main user.
- Information Snippet: This Field will allow the Admin to relay information to the end user (Announcement, Disclaimer, Section information, etc.).
- Line Break (for New Section): Adds a solid horizontal line to the form.
Payment
These fields are used to capture payments to specified charges.
- Choose Multiple Quantities: A field that will allow for multiple drop downs each will allow the end user to choose a quantity of a specified amount for a specific charge:
The end user will see dropdown menus from 0-10 for each type created in the field. Each type will allow you specify its name, monetary value and charge type. - Choose a Quantity: functions like above but with only one drop down menu to show quantity, Can choose max quantity defaults to 0-10.
- Donation Dropdown: This field creates a drop down menu:
The menu is automatically populated with a list of donation types checked off in Admin menu->My Lists->Charge Types: - Enter a Quantity: This field displays an editable field that only accepts a numerical value from the end user that represents the quantity to pay for a specified amount for a specific charge.
- Enter in an Amount: This field displays an editable field that only accepts a numerical value from the end user that represents the amount to pay for a specific charge.
- Multiple Checkboxes: This field will create a check box next to each type specified by the Admin. each type will consist of a specified amount and charge type. a maximum choice of checks can be specified in the field options.
- Pulldown Selection: This field creates a pull down menu of the types created by the Admin (specific amounts and charge types) so only one can be chosen.
- Radio Selection (Choose 1): All options (of the types created by the Admin with specific amount and charge type) are displayed to the end user but only one can be selected via the selection bubble on the left.
- Single Checkbox: A field that allows for only one Admin specified amount to a specific charge type.
- Total Display: This field will automatically tally up the amount of all selections made by the end user using any of the Financial and Payment fields above.
Person Fields
These fields will can be used to draw information from or input information to people on an account.
Referencing will draw information already saved in a 'Person' tab belonging to the Primary, Secondary or the Person logged in. Action offers the ability to save information inputted into the field into the "Person" Tab.
The fields in this section include:
- Bar/Bat Mitzvah Date: This field functions like the "Date and Time" field under General, it will draw from/save to a person's information tab.
- Bar/Bat Mitzvah Location: Is a single line of text field that will draw from/save to a person's information tab.
- Birthday: A date field that will draw from/save to a person's information tab.
- Business Name: will draw from/save to a person's information tab. Max Length defines how many characters can be inputted to the name.
- Business Phone: will draw from/save to a person's information tab. Max Length defines how many characters can be inputted to the number.
- Business Position: will draw from/save to a person's information tab. Max Length defines how many characters can be inputted to the name.
- Email: will draw from/save to a person's information tab.
- First Name: will draw from/save to a person's information tab. Max Length defines how many characters can be inputted to the name.
- Gender: will draw from/save to a person's information tab in the form of a drop down menu.
- Hebrew Father First Name: will draw from/save to a person's information tab, a pop up Hebrew keyboard is included in the field.
- Hebrew First Name: will draw from/save to a person's information tab, a pop up Hebrew keyboard is included in the field.
- Hebrew Mother Name: will draw from/save to a person's information tab, a pop up Hebrew keyboard is included in the field.
- Last Name: will draw from/save to a person's information tab. Max Length defines how many characters can be inputted to the name.
- Maiden Name: will draw from/save to a person's information tab. Max Length defines how many characters can be inputted to the name.
- Middle Name: will draw from/save to a person's information tab. Max Length defines how many characters can be inputted to the name.
- Mobile: will draw from/save to a person's information tab. Max Length defines how many characters can be inputted to the number.
- Parsha: Allows for the selection of the Parsha via a dropdown menu.
- School Name: will draw from/save to a person's information tab. Max Length defines how many characters can be inputted to the name.
- Title: will draw from/save to a person's information tab. Max Length defines how many characters can be inputted to the name.
