Account Fields
Account Fields are specific to the data saved in an account and auto-populate if a user is logged in when completing the form based on the information in their account. If the Account Field in the form is a field not completed in the account, or someone not logged in is completing the form, the data entered in the form can auto-update the account when "Apply to Database" is selected.

Enrollment Fields
Enrollment fields are used when signing up to pay for an enrollment. The enrollment is selected via drop down menu within each field. Additionally, an enrollment with or without payment can be selected.

Additional General Field Options |
Choose the enrollment being incorporated into the form. If Enrollment with Payment is chosen, the following fields must be completed:

Choose 1 Radio | Given multiple items within the enrollment selected , all items are shown in the field but only one item can be selected. |
Choose 1 Select | Given multiple items within the enrollment selected, items are selectable via a drop down menu and only the one selected will be visible in the end. |
Choose Multiple |
Given multiple items within the enrollment selected, a drop down menu with a check boxes next to each item. Only items selected will be visible in the end. |
Choose Quantity |
Given multiple items within the enrollment selected, each item will have a dropdown menu with the option to choose from the quantity available from that originally established when the enrollment was created. The remaining slots will also be visible next to the item name i.e. 'item name' 5 of 7 left or 'Item name' 7 open. |
Event Seating & High Holiday Seating
These fields are associated with the Event Seating and High Holiday Seating (not Events) chosen in in the drop down menu in form options at the bottom. Only transaction information is saved in the Form submissions, the rest is saved in Event Seating.

Additional General Field Options |
Choose the highest quantity of seats that can be chosen in one submission as well as the seat locations. Seat Locations can be chosen by room (Sanctuary, Social Hall, etc.) and by holiday (Rosh Hashanah, Yom Kippur, Both, etc.) If Choose Number of Seats with Payment is chosen, the following fields must be completed:

Financial Fields
These fields draw financial information from an account, public users will not see these fields and these field will be skipped. The fields in this section include:
Field | Description |
1. Donation Based on Contributed Amount |
Allows selection of specific charge types to display what was paid in the past to the selected charge types. |
2. Require Payment of Due Listed Transactions |
If selected, this field will be auto-required. It displays the open balances of any/all charges on the account within the date range specified. If there is no Balance, a suggested amount toward a specified charge type can be recommended. |
3. Require Payment of Total Due Amount |
If selected, this field will be auto-required. Rather than listing all transactions with an open balance, it will list the total due on the account. |
4. Suggest Payment of Due Listed Transactions |
Functions exactly the same as Require Payment of Due Listed Transactions except the payments are marked as suggestions and can be edited. |
5. Suggest Payment of Total Due Amount |
Functions exactly the same as Suggest Payment of Due Listed Transactions except instead of displaying each transaction it only displays the total amount. |
When a Financial Field Type is selected, the following options become available.

Select the date range the system will search and populate transactions based on. |

Choose a dollar amount for the system to search and populate transactions based on. |
3. Charge Types |
Choose a charge types for the system to search and populate transactions based on. Populated from Admin > My Lists > Charge Types. |
4. Payment Status |
Select between transactions that are:

5. If 0 - Label |
If an account has not had a past transaction with a charge type, this field can be used to suggest an amount to pay or donate. When something is entered in the label, additional fields for Suggested Amount and a Charge Type (field next to "using") and an additional text description. |
6. Suggest Multiplier |
Suggests an additional amount to add onto previously paid or charged transaction. |
General Fields
These fields will gather information that the user inputs into the form and stores it only within that form submission. It will not be saved to an account or person record. The General Fields that have more than the regular options will be detailed in the chart below.

Payment

Choose Multiple Quantities |
Field allows for multiple drop downs and for the end user to choose a quantity of a specified amount for a specific charge. The end user will see dropdown menus from 0-10 for each type created in the field. Each type will allow you specify its name, monetary value and charge type. 1 - Label: Label for the dropdown 2 - Value: How much per item 3 - Name (Unique): Internal name 4 - Charge Type: Choose a charge types for the system to search and populate transactions based on. Populated from Admin > My Lists > Charge Types. 5 - +: Add additional dropdown items. |
Choose a Quantity |
Provides one drop down menu to show quantity, Can choose max quantity defaults to 0-10. Use Value field for amount per quantity and choose a charge types for the system to search and populate transactions based on. Populated from Admin > My Lists > Charge Types. |
Donation Dropdown |
Creates a drop down menu that mimics payment.php donation dropdown. Populated from Admin > My Lists > Charge Types. In Charge Type Settings, Show on the Donation Dropdown must be selected. |
Enter a Quantity |
Allows user to enter a numerical value. Number entered in this field will be multiplied by the value for a total. Use Value field for amount per quantity and choose a charge types for the system to search and populate transactions based on. Populated from Admin > My Lists > Charge Types. |
Enter in an Amount |
Allows user to determine how much to pay/donate. Charge Type for payment allocation is set by admin as in other options above. |
Multiple Checkboxes |
This field will create a check box next to each type specified by the Admin. Each type will consist of a specified amount and charge type. a maximum choice of checks can be specified in the field options. |
Note on Transactions Added (Only 1 Per Form) |
Allows a text box to add a note to the transaction from the user. |
Pulldown Selection |
Pulldown menu of the types created by the Admin (specific amounts and charge types) so only one can be chosen. Option to provide a search box for the Pulldown options. Additional setting to determine if the Value (amount per item) is fixed or a suggestion and can be changed. |
Radio Selection (Choose 1) |
All options (of the types created by the Admin with specific amount and charge type) are displayed to the end user but only one can be selected via the selection bubble on the left. |
Single Checkbox |
A field that allows for only one Admin specified amount to a specific charge type. Options are the same as Enter a Quantity. |
Total Display |
Automatically tallies up the amount of all selections made by the end user using any of the Financial and Payment fields above. Recommended for use. |
Person Fields
These fields will can be used to draw information from or input information to people on an account.
Each field has options for referencing and action. Note that information for children cannot be applied to the database.

The only field that differs from a general form field is Email as it has a checkbox to select which email field in the form should be used for the confirmation email.
